CASE MANAGER
TITLE: Case Manager
REPORTS TO: Clinical Director

LOCATION(S): BTC OTC Vernon and BTC OTC Show Low
JOB SUMMARY: Under the general direction of the Clinical Director, the Case Manager provides comprehensive support and coordination of care throughout the addiction recovery process. This position assesses client needs, connects clients to appropriate resources including housing, employment, medical care, and legal services, and monitors progress toward recovery goals. The Case Manager collaborates with the treatment team, advocates for clients’ needs, facilitates communication between clients and external services, and ensures continuity of care. This role utilizes a GMHSU integrated treatment approach to help clients overcome obstacles and maintain long-term sobriety.
ESSENTIAL JOB FUNCTIONS:
- Accomplishes clients’ care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches.
- Admits new clients by reviewing records and applications, conducts orientations, needs assessment and coordinates medical record requests from previous providers.
- Coordinates wellness checks, doctor appointments, and other necessary services to ensure continuity of care.
- Monitors cases by verifying client attendance, observing and evaluating treatment responses, advocating for needed services, and intervening in crises.
- Maintains current contact lists of community resources and coordinates referrals to housing, employment, medical, legal, and social services.
- Assists clients in establishing/re-establishing identity with state and federal departments (SSA, ADOT, Vital Records, DHS, HHS).
- Communicates effectively in writing, interpreting client legal commitments and court-related communications including completion letters and status reports.
- Sends FMLA paperwork and return-to-work notices; assists clients with employer communications.
- Prepares clients for discharge by reviewing and amplifying discharge plans, coordinating post-discharge requirements, and orienting family members.
- Provides 90 hours per month of case management time, managing coordination in scheduling, setting appointments, and organizing client services.
- Documents critical steps and progress within the recovery program, maintaining accurate and up-to-date records in the EMR system.
- Facilitates groups as directed, with bi-weekly supervision from the Clinical Director or Assistant Clinical Director.
- Conducts client surveys including outcome surveys, satisfaction surveys, and other tools as directed.
- Contributes to department and organizational success by taking initiative, embracing new challenges, and seeking opportunities to add value.
QUALIFICATIONS, EDUCATION, EXPERIENCE, AND OTHER REQUIREMENTS:
- Twenty-One Years or Older
- Minimum Education: High School Diploma or GED required
- Meets the Behavioral Health Technician (BHT) qualifications; meets or exceeds one of the following criteria:
- Minimum type and duration of experience of 2 years of related behavioral health experience, and/or training or equivalent combination of education and experience
- High school diploma or equivalent required; associate’s degree or higher preferred
- Preferred: Experience with case management, AHCCCS benefits, and community resource coordination
- Proficient in basic computer skills and EMR systems
- CPR/First Aid Certification
- Nasal Naloxone Opioid Reversal Training
- Two-Step Test Freedom from TB
- Must hold valid and active/unrestricted Arizona Driver’s License
- Adherent to the BTC Code of Ethics and Standards of Conduct
- Must pass a background check and pre-employment drug screening
- Knowledge of various recovery modalities (e.g. Twelve Step Recovery, SMART recovery, etc.) and the disease model of addiction.
- Knowledge of case management practices, GMHSU integrated treatment, and community resources.
- Knowledge of AHCCCS benefits, RBHA/TRBHA systems, and health plan benefit structures.
- Strong problem-solving skills, time management, and organizational abilities.
- Excellent verbal and written communication skills.
- Knowledge of HIPAA, 42 CFR Part 2, and client confidentiality requirements.
- Knowledge of software programs for personal computer use, especially word processing, email, spreadsheets, and EMR systems.
- Knowledge of AHCCCS benefits enrollment, community supports, RBHA/TRBHA, and health plan benefit systems.
- Ability to communicate effectively with families, courts, probation officers, and community agencies.
